I've tried fishing in lava and in magma thresher's schools but no luck with getting one. Tried for about 1 hour (30 minutes in lava, a bit more from school to school). Still don't know how to get one

Hunter here, tried eating the fish with aspect of turtle and survival of the fittest to reduce damage. Fish disappeared, got the food debuff, but no achievement.

While eating the fish, you will take damage based on your maximum health.
This means you can unequip all your gear, start eating, and any healer (even without high level gear) can keep you alive through the 20 seconds without breaking a sweat.

as resto druid this achievement was pretty easy- just used efflo+ all of my hots, defensives and I ate the fish, at the end I was like on 50-60% HP, no issues at all.

Prot Warrior didn't even break a sweat. "Second Wind" healed it all.

The Cobalt Assembly wild arcana Arcane Bravery makes this completely simple. Just be sure to wait until you're completely finished eating--getting the flounder buff is only halfway done.

Paladin's don't try and avoid the first 8sec with Bouclier divin, it stop the de-buff applying, so no achievement...

Warlock, started eating - used dark pact - thought I'd get it but had to pop unending resolve. Survived, no achievement.

Was able to get the achievement on a warlock with 432k max HP and Demon Skin by making a naked gear set, popping Dark Pact and then Healthstone to immediately get back to full health, switching to the naked set, and eating the fish. Switching made the Dark Pact shield 230% of my max health, plus another 15% from the Soul Leech shield, which was enough to absorb all of the damage.

Pro Tip! Strip down nekked and queue for a Follower Dungeon. Eat it right away while the healer is tossing out buffs and they will heal you all the way thru the achievement. Leave dungeon.
