Darrowshire is in the Eastern Plaguelands. The doll is laying on the ground, near the wheel on the right as you're facing the meat wagon.

Once you have the doll, return to Caer Darrow in the Western Plaguelands and turn this item into Sammy to receive Bougie répulsive à fantômes. He's standing by the fountain.

Note: You will only be able to see this NPC if you have Essence spectrale equipped.

This was confusing, so ignore the other comments, this is found in Eastern Plaguelands, you're currently in western. Exact coords are: 35.7 83.7. I will post a picture of the exact Meatwagon.
