I've farmed this since the very first day of legion, in total i've killed over 70 rares and still nothing.
In total i've farmed the mobs for over 10 hours now ...

About needing to be 110 or not, I think its best to assume you might aswell wait till 110 and then go for it, you'll loose plenty of time

Confirming that this does drop from Underbelly rares; just had it drop for me off the Crazed Mage rare while PvP was enabled. I just reached Level 4 artifact knowledge about an hour ago, that might have something to do with it.

Confirmed drop with AK4 after around 20 rare kills.
Dropped off non-wanded Screek (so wand shouldn't matter).
Guards were on duty(so it shouldn't matter).
PvP was off (PvE realm).
Combat spec (with loot spec set to Assassin) when I got it (so current spec doesn't matter).
Kingslayers was at Artifact level 1 (so AL doesn't matter).

Just Got mine from crazed mage last night. I have AK 4, Guards were on, and I had a bodyguard. Got it after about 25 kills or so. I also had just killed the last boss in The Nightmare. Not sure if that matters (It probably doesn't)

Now that it was found that you need AK 4 for this, I wonder if you can get it to drop while in another spec like the Subtlety artifact appearance does, or if you have to be specialized as Assassination.

Been farming this for 2 days, i have 5 AK, still nothing, it pisses me off even more that people keep killing me (i know it's a pvp place, but i hate that i have to go to pvp to get an artifact)

I can confirm aswell that it drops from sewer rares (personally i got from the rat rare) after 17k slightless eye :)

ALSO i think you should have the level 5 artifact research to get it

For those that already have the hidden appearance, Here's how to track your hidden artifact appearance color variants through achievement progress:

/run AddTrackedAchievement(11152) - Complete 30 dungeons using a hidden artifact appearance. (This achievement does not display correctly)
/run AddTrackedAchievement(11153) - Complete 200 world quests using a hidden artifact appearance.
/run AddTrackedAchievement(11154) - Kill 1,000 players using a hidden artifact appearance.

Check all your hidden artifact appearance progress using this script

I've been farming close to 100 elites in Underbelly and have yet to see it drop, however, it dropped on the FIRST kill after I've used Wild Mana Wand on an elite.

I'm not sure if this is just pure coincidence. Would be great if someone could try this to confirm or refute.

Just got this this morning, dropped from Crazed Mage (seems to have the best drop rate based on comments here). Me and another rogue were duking it out all morning in the underbelly, but the second I had it drop I linked it and he went bananas. He followed me to our class hall so we could see the requirements, skins, etc.

Playing with a transmog for the default set right now - it's a hard one to match. I'm using the AQ Tier 2.5 set, Deathdealer's and it looks pretty sweet.

Dropped from the first underbelly boss I killed after I reached AK4. It was Xullorax (upgraded with a Wild Mana Wand). I was specced Outlaw when it dropped.

Finally got mine to drop off Segacedi. Just as I was about to lose hope on ever getting them it dropped at the exact same time that I earned the 20,000 Sightless Eyes Achievement. I don't know if the achievement increases the odds or not, I just thought it was interesting enough to share.

Don't let the recorded droprates discourage you from farming this, remember they also count other classes looting the mobs that can't loot this item.

Finally got this today after spending enough time in the Underbelly to get the Ratstallion mount from only killing bosses.

I'm not into PVP, so I would buy a guard from Raethan (only cost 5 gold for ~ 5 minutes of protection) during the PVP phase. I would also run to buy a guard if everyone was close to killing a boss and the guards were pulled away. Sometimes players will pay Raethan to remove the guards and kill everyone surrounding the boss. If I died in this situation, I couldn't loot the rare when I got back.

Finally got mine. I went from basically zero Sightless Eyes to 16,997 before I got the drop off the Crazed Mage. I might as well stick it out until I get the mount as well.

Today I went to the Underbelly to get my hands on the items from the Diablo anniversary and was farming rares in the mean time because I wanted the Cypher. After approximately 2 hours I had the appearance dropped for me from a Thieving Scroundrel.
And I know it doesn't change anything but my AK is 24.

Just dropped off the crazed mage while I was in outlaw spec. PVP was not enabled. I'm at AK 25 and was killing goblins for the Diablo toy. Killed a total of 6 elites over the course of an hour.

Nabbed this off a wanded mana seeper yesterday afternoon, took about a week of trying on and off. I learned very quickly that you want to keep a bodyguard at all times to prevent the pvp flag that you get if you hire one after getting the free for all tag. The pvp flag still let folks attack even with the guard. I was also summoning the crocs in between rares, as they have a listed chance to drop this, and it's an easy way to get eyes for returning the guards.

Can confirm this item drop rate is way to rng. Have enough eyes to not just buy 1 rat mount but now two. Maxed out in everything so please don't yell out "you need to be lv4"

They seriously need to fix this drop rate. Meanwhile others can just go to a vendor and buy them or click a portal and bam they got it. No effort needed at all.

When I eventually do get it I basically start from scratch on horde kills, dungeon clears and WQ completed. Ive past those requirements up as well.

Ill probably get it from pvp prestige before I get the dang drop.

Just got mine after about 100 kills (didn't know about the cap on Eyes, so can't count precisely).

The weird thing is: while farming, I've been surfing the net on the topic and walked upon a guy who claimed he read somewhere that reading artifact history prior to farming helps. Personally, I think this looks like nonsense, but why not try? I went to the Class Hold to read the book, and after that there was 2 or 3 people in the sewers so the mobs wouldn't spawn? so I left. Today I log in and the cypher drops from the first kill.

I still think it's totally unlikely that such a mechanic would exist, but if SOMEHOW that's the case, that would be… funny.

I achieved the drop after four rare kills, in the Underbelly.
I've heard it taking people 2 weeks before.
Either they just had bad RNG, or there indeed was a change.

There was a change for most hidden's being droppable on Argus!
There is a possibility this was changed as well!

I haven't seen any of the top comments mentioning this, but this appearance can also drop from the chests that spawn. I looted this out of the red and gold chest - the largest one out of the horde of chests.

Not sure how hard it is to get these days but I got it in about 5 minutes of hanging around whilst the guards were on duty. Killed a couple of thieves and eventually the Mana dude dropped it for me. Wasn't hard at all to get. I really like this skin. Other than the Manablade this has probably one of the coolest looks.

If you don't want to farm, and you aren't big into PVP but it's also not the worst thing in the world to you if you're flagged for a couple of minutes, my suggestion is:

Wait until a non-peak time and there aren't a ton of people online (maybe that doesn't matter much due to phasing; I'm not sure)
Talk to the guard captain and tell him to call off his guards (costs 50 eyes—if you don't have eyes, get them from the alchemist)
Immediately run into the side room where the black market auction house is
About half of the time there will be a big glowy Underbelly Hoard in there, and the people in there are probably there to fish or use the BMAH, not fight... so take it (it'll become active a few seconds after the guards leave)
(If there isn't an Underbelly Hoard there, it's in the main room, so you'll need to either fight for it, or just leave and try again in 15 minutes or so.)

The Underbelly Hoard appears to maybe have a 100% drop rate for the rogue and shaman hidden artifact appearances, and I got both of mine in a few minutes this way.

In case anyone was wondering. I was bored in the underbelly on my rogue and not after this appearance because wowhead told me it was off boss drops, so I was looting chests. Not that long later this dropped in the chest that gives 100 sightless eyes. So if the underbelly on your server isn't that full, and no one's after chests, or you can just kill those after the chests, just hang out, loot the chests and eventually it should drop.

Confirmed drop today (2019-03-18).

Dropped after killing my 3rd rare in The Underbelly. (weird enough it dropped it as 3 items, and yes, you could use them all, even though it didn't do anything but the cinematic after the first one unlocking the actual skin).

I was level 120, and I did NOT wear my artifact weapon whilst killing the rares, however you do need to have the artifact equipped when you use the hidden artifact appearance item.

I believe it cannot drop before 110 despite Artifact Knowledge not being around anymore. As I replied to Therakyion's comment: Between levels 102 and 109 on my Zandalari Rogue, I gathered 10k sightless eyes, almost exclusively from rares, which most were buffed with a wild mana wand, but no artifact skin drop. I reached level 110 and the skin dropped on my first rare.

As of patch 9.1.5, just leveled a rogue finally (lvl 53) for first time on my account through Legion. Finished up almost all of the rogue campaign stuff (not all). Got the drop after killing Domino in the Underbelly. I just killed a rare prior to this without Les Régicides equipped, no drop. Equipped them and Le cryptogramme des os brisés dropped!
