I'm going to go out on a limb and guess that the drop rate is only listed at 1/4 because not everyone who's looted the chest had the quest for the artifact at the time, like a lot of the other quest objects here on Wowhead - you know, the various heads and what not.

Not that anybody cares about the lore anymore, but the Nathrezim were a clan of demons chosen by Sargares, with their homeland is somewhere in the Twisting Nether. Varimathras, Mephistroth, Mal'ganis, Tichondrius, Dalvengyr, Balnazzar, Anetheron, and Detheroc, are members of the Nathrezim.

Hm. I've been lugging this around in my bag. As I recall, it was a turn-in item for one of the heroic dungeon quests that are no longer available - and I no longer have it in my quest log. Any suggestions what I should do with it?
