I find it kinda wierd that a quest item doesn't have 100% drop rate.
Most characters run Zul'Farrak more than once, but you might actually get unlucky enough to never see it drop..

I've done one ZF run with the quest, and no scale dropped. First I wondered if it was because I summoned him without the http://www.wowhead.com/?item=9240 (which is possible now, through patch), but now it turns out that the drop rate is to blame. trannsvaal's comment is therefore a bit misleading.

The Tiara and Scale are still not guaranteed drops.
