200 uses with 43 misses without any hit modifiers.
~21.5% miss rate.
~78.5% hit rate.

Tooltip says 90% hit, but after seeing its performance in pet battles I started keeping track. RNG is RNG and all, but take my experience however you will. I think blizz should make sure it's hit % is at the listed rate.

This is the WORST ability! It's so unbalanced in PVP battles, it does huge damage and disrupts your pet matching, super annoying.

They either need to tone the damage down, or remove the secondary effect. A huge nuke which also stuffs up your opponents team significantly is just too OP.

Just wanted to post the other pet swapping spells. To my knowledge, there are only three abilities so far:
Death Grip: Currently only used by Creepy Crate, Giant Bone Spider and Crawling Claw.
Nether Gate: Currently only used by Fiendish Imp and Netherspace Abyssal.
Sweep: Currently only used by Enchanted Broom.

If anyone knows of any others I've missed please post.

Yeah Blizzard apparently agrees that these abilities are OP:

Death Grip cooldown increased to 4 rounds (up from 3 rounds), damage has been reduced by 50%, and hit chance has been reduced to match similar abilities.
Nether Gate damage has been reduced by 33%.
Sweep damage has been reduced by 33%.
