Received this today, and I am not sure how it applies to my loot specialization. As a Holy Priest I rarely, if ever, attack anything during a fight, especially in WoD thus far.

I am a resto druid that received this item and was confused about the proc as well. I tested it today on my garrison training dummy and it does not proc with heals. It did proc on damage rather quickly. Maybe I could throw up a moonfire here and there for the proc but that just annoys me. I hope this is not going to be the trend this xpac. I prefer on heal procs.

I can 100% say that it will work with heals IF you are in legitimate combat.

It will not work with a training dummy (as of Nov 28th).

It will show up with a green blood boil type buff.

Works for HOTs and direct heals.

I got this today as a Restoration Druid, was confused so looked here to see a lot of people saying it didn't work for healers, so I decided to test it out by going and agroing a mob to put me in combat.

The buff ticked straight away from my first Rejuvenation tick.
