Doesn't have to be set on heroic. There is a Void Spawn that pats into the room, so wait until it's out before grabbing the item.

Easy to stealth/camouflage in, as someone told me. However that person neglected to mention that it is on a different floor. Same place as indicated on map, but one floor down. Then hearth out. I tried doing it in an instance group a couple of times, but had no luck getting it as no one wanted to wait for me.

You can no longer pick up the item and drop from the group, since these quests now require you to defeat the final boss in the appropriate dungeon.
