Curious:

Since looting the head of any given mob in WoW is basically the game's way of saying said mob is forever considered 'dead'...will we be fighting a headless Rivendare in Stratholme?

Or is he the TRUE Headless Horseman?

For whatever reason, you can loot Baron's head while in a raid group. I managed to loot it while waiting for a summon in H Sartharion group.

Believe it or not, the level cap used to be 60.

The droprate is indeed 100% if you are on the respective quest.

The loss of his head to alot of players dosen't seem to have stopped him from coming back in wrath.

Not sure where I read it, Wow-wiki I do believe, it is suspected that the Baron Rivendare in old Stratholme isn't even the true Baron but an impostor. I think somewhere there's a journal of some sort that details something along the lines of a cultist taking his spot. Or I could be mistaken and its the impostor in Naxx.
