This item can be found only (so far as I know) in the Siege of Boralus Mythic Dungeon unlocked by following the Alliance story in Kul Tiras. It will appear as a regular yellow "!" in an out of the way corner of the instance. I believe it was between the second and third bosses. I was expecting the turn-in for the quest to be in Boralus after finishing the instance, but it's actually in Stormwind Keep in the Library: to Kayla Mills.

Also, an FYI (since it caught me by surprise), that turning in the quest plays a cinematic of sorts. Not just NPC dialogue either, but a full cut scene. I won't say what it is, so it's not spoiled for those who are concerned about it, but it is relevant to the story of BfA and it really tied the Alliance story together well for me (even though I had seen it before out of game).
