Why wouldn't you cleanse it if the toy does the same thing?

Just so people are clear, you must first complete a quest chain. To obtain the item to begin the quest chain you will want to wait until the world quest Naga Attack! is up. I found mine up in Zuldazar. Kill and loot Naga during this WQ and you will obtain the quest item Médaillon d’Azshara. At the end, if you cleanse N'Zoth's gift you will get the toy, if you decide to keep it you will have a permanent (presumably) buff, and you can see others who kept the gift. I advise you cleanse the gift on an alt to get to the toy, but keep the gift on your main. Here is a good write-up on Wowhead: https://ptr.wowhead.com/forums&topic=291069.

To Start this Story line You need find Médaillon d’Azshara, this is droping from mobs on World Quest Une attaque de Nagas ! ( on all of them ). After you will get Médaillon d’Azshara you will start storyline.

Just a warning; Blizzard states that this gift will have its purposes in the future. So if you cleanse it off you by completing the quest, this Toy may opt you out of that special event. I encourage you to use another character and cleanse them so you can see the aftermath of both events.

This will not cause npcs to react to you, it is purely cosmetic and everyone will see it and the buff. Unlike the real eye/gift which is a hidden buff that is only seen by other players with the real gift and that version causes unique reactions from npcs.

If you complete the quest for this on one character and don't on a second you can then use this toy on the second character and click off the buff to temporarily hide the permanent eye from your head (and return your head piece).

Just tested it and this DOES NOT allow you to understand Shat'yar.
Unlike the true gift which does.

Just in case anyone else wasn't sure of it: you WILL get the toy if you complete the quest now, in 8.3 (and likely later).

A few comments on this quest chain sounded like it'd only work if one completes them before the next major patch. Not sure if it's still possible to begin the quest chain, but I had in my quest log ever since 8.1 and only got myself to complete it now - yes, got the toy.

Well I think this is a mystery. It is now 2022 and I still have the eye attached to my head which I got a month ago. So... if WOW had something in mind for the future then it still hasn't happened. That's 4 or 5 years ago now. So... I guess that, when you get this thing stuck to your head, maybe we each have to wait a certain amount of time, depending on when you received the gift. But I probably won't even find it worth the time wearing it if I don't care for the ending. Only a mount would be worth it. And even then, I don't like those ugly monster mounts with 3 heads, or 10 legs, or "ONE EYE". I can't find anywhere what happens if you keep the eye.
