Yiphrim the Will Ravager is spawned as part of the Summoning Ritual event during Black Empire assaults in Neferset, Uldum. The Summoning Ritual event is completed by killing all the Acolyte de N'Zoth and then closing the summoning portal. This event is special; after it is completed a number of times a set of three rares will spawn around Neferset at (50,78), (50,88) and (55,79). The possible rares are:

Aphrom l'Incarnation de la folie
R'aas le Dévoreur d'anima
R'oyolok le Bâfreur de réalité
Shugshul le Gobeur de chair
Yiphrim le Ravageur de volonté (drops Déguisement k’thir bon marché!)
Zoth'rum le Pilleur d'intellect

You can only close the portal once a day, so you will have to wait for others to complete the event to trigger the rares.

You can farm it in Raid group and click the crystal as often as you want!

You do NOT need to kill the adds because the rars can spawn without killing any add, i see it very often.

Easiest way is to farm with a raid group an click the crystal with a druid so you never had to kill a mob.

I do not know if you must kill the rars to get a new spawn, my farm groups cleared all rars before we continued clicken the crytal.

Today we convinced a bunch of characters/players to help with this to attempt to spawn the rare which has a chance to drop Déguisement k’thir bon marché.

We first got the rares to spawn at 2:28 PM after some unknown number of clicks on the summoning portal.
We finished killing all the 3 spawned rares at about 2:32 PM.

We then repeatedly summoned multiple characters to click the summoning portal one by one following this.
Having clicked the summoning portal every time it spawned on at least 15 different characters, the rares finally spawned again at 3:02 PM.
We finished killing the second set of rares at about 3:05 PM.

Testing then with just a two-player raid group, the summoning portal is able to be repeatedly-clicked by the same player (assuming they have not already received quest credit for clicking the portal).

A 3rd set of rares spawned at 3:44 PM when clicked by a player in a raid group. Yiphrim finally spawned in this set, and all 3 were fully killed around 3:51 PM.

Then at 3:59 PM a 4th set was spawned, again when clicked from a raid group on the 3rd click since the previous set was killed and again with Yiphrim. This was only 8 minutes since the previous set was fully killed. We killed the 4th set of rares sometime after that...

And at 4:07 PM a 5th set of rares was spawned only 8 minutes since the previous spawn, and less than a couple minutes from even killing the previous set.

TL;DR From this research it seems that currently the following may be accurate:

Any click can spawn the rares, even when in a raid group, and without cooldown from a previous rare spawn it's just a low chance or possibly increases with each click? You know, the usual RNG
All previous rares probably need to have been killed Hard to disprove unless they can spawn without killing all prior rares, which has not yet happened

I came here to look at how to get this toy and was totally confused. So I went and check out Neferset City during an N'Zoth assault.

I saw Shugshul le Gobeur de chair up but didn't touch him.

I went over to where Yiphrim the Will Ravager is marked on my Tomcat's Tours addon (udlum 50, 78) and I saw a bunch of Acolytes channeling around a portal. I killed them all and Yithrim showed up.

I started a lfg group and one person joined, we both killed him and both got the toy.

No idea if I got lucky on Yithrim spawning or getting the toy. But I sure can't understand the instructions given on this page.

Finally dropped.

You can only click the crystal once, so I used the 'form a raid' trick to allow me to re-click constantly (see other posts for instructions) getting bored with nothing spawning, I assumed the rare spawn was random, so I decided to camp and watch other people click.

Turns out when you click the crystal, the random rares can spawn in three locations. If you click and nothing happens, that just means the rares are already up in one of the other spots. I didn't realize at first, so who knows how long I re-clicked the crystal over and over, meanwhile Yphirim stood around at the other spots, waiting?

I solo'd most rares, except R'aas, which needed help bringing down. After summoning and killing the rares over and over, Yiph finally appeared at one of the other spots.

I already looted him on my main that day (no toy), so I quickly logged onto an alt and killed him. He dropped the toy. Yay, octopus friend!

Got it on my first kill.

Standard approach:
1. Make sure that the portal event is up
2. Make sure all 3 rares are dead
3. Make sure that you are in the raid group
4. Click the portal while mounted, repeat until it spawns the 3 rares
5. Hope for the best, if the rare spawned, or kill all 3 rares (they have 500k+ life each) or swap to warmode ON/different shard and repeat

I'm just going to expand on the below post from 2022
1. The Assault must be up for that area
2. Make sure that the portal event is up in one of the 3 spawn points. Takes about 2 minutes for the portal to respawn but its not up every day
3. Go around and make sure there are no Rares in the 3 spawn points
4. Go into 'Group Finder' > Premade Groups > Custom > Start a Group > (give it a title discouraging others to join)
5. Log off and log onto an alt - find the group on group finder and click to join
6. Log back into your toon chasing the toy and accept the invitation
7. Sadly you must log back into the alt to accept the invitation
8. When you log back into the toy chaser you now have an alt thats 'offline'. Thats ok. Right click on your icon top left and 'convert to raid'
9. You now have yourself a raid that shouldn't expire. Membership of 2
10. Find the portal and click. About every 6 to 10 clicks the 3 Rare's will spawn. If you are lucky one of them will be 'Yiphrim the Will Ravager'. Kill him first to see if he drops the toy (before someone else wandering by kills him before you get there. No toy? Sorry. You only get one chance/day per toon.
11. If Yiphrim does not spawn then sorry. You need to kill all 3 Rare's and go back to the portal and try again
12. After 2 hours with this method I managed to get Yithrim to spawn 3 times but he didn't drop the toy (2022)

Edit: I finally came back to this today (August 2023) and after about 6 hours the toy finally dropped on my 4th toon for the day. Remember you can only hit Yiphrim once per day per toon
I killed Yiphrim approx. 10 times over the journey. Just getting him to spawn is a pan
