Looted from a treasure box in Revendreth

Chordy found this for me. Chordy's loot table is the same anywhere in the Shadowlands, so it's just about finding the best spots to farm. Popular places include: Oribos on the flight master's floor, the lap area of the big Primus statue, Hero's Rest in bastion, on top of any big mushroom in maldraxxus, etc. ideally somewhere open and flat. I prefer the Primus lap area. The drop chance is excruciatingly low, but there are multiple rare items he can give you for cosmetics. I've been finding about 1 rare cosmetic item per half hour I spend, roughly.

Also got it today while doing Toothpick's weekly quest in Ardenweld, from chest which spawned from her ability.

This can be found in a secret treasure that's located in Darkwall Tower (Revendreth), in a crypt behind a bookshelf that disappears when interacted with.

found it in revendreth while doing the quest with toothpick, hitting ground revealed a chest with the hat in it

This can be found in the Secret Treasure in Revendreth at /way 42.50, 50.39. The chest is downstairs in a crypt (entrance at /way 41.95, 50.04). Fly to the Charred Ramparts flight point and take the lift just NE of the flight point (at /way 42.04, 46.74).

You get access by clicking the "non-descript book" on the shelves in the room on the right. Please note that the Secret Treasures are only up during the L’appel de Revendreth.

- please note, that as for all of the secret treasures, it is random which ones are up during L’appel de Revendreth. You can however check the map to see if there is a chest at the listed location.

Looted this today from the Secret Treasure in Revendreth mentioned in other comments while I was Night Fae. My stitchyard is level 5. It seems that your current covenant doesn't affect whether you can loot this item.

Revendreth Buried Cache & Ardenweald Buried Cache

How to collect Buried Cache:

Check if you've already collected either of the two Buried Cache through these scripts(if true, then it can't spawn today):
Ardenweald Cache: /script print(C_QuestLog.IsQuestFlaggedCompleted(62240))
Revendreath Cache: /script print(C_QuestLog.IsQuestFlaggedCompleted(62247))
The "non weekly-quest" Cache resets daily, make sure to get one cache every day.
Pick up Toothpick from /way #1536 54.8 67.8 Toothpick
The weekly quest is not a requirement for the Buried Cache to appear.
Move to Ardenweald or Revendreth and use Toothpicks special ability World Shaker at the following locations:

The cache doesn't always spawn from the first time you use the ability so make sure you use it a couple times before moving to the next location.

I haven't personally acquired the big floppy hat yet but I'll update this post when it does happen. :)

After many a days of looting the Buried Cache I eventually got the secret treasure in Revendreth spawn and that's where I finally got the Big Floppy Hat from.
Sadly this means I can't confirm wether the hat can drop from Buried Cache or not.

Was anyone able to get this item recently? I have spent maybe several months now, looting Buried Treasures daily and Secret Treasure every time it was up. I suspect Dragonflight pre-patch or the launch itself may have bugged it. Last one I need for Abomination à la mode.
