To summon Exos, héraut de la domination you need combinate 3 items to create Appel de la domination.
I got all items on my first try.

Gravure de domination : deuil drop from Apholéïas, héraut du deuil.
Gravure de domination : chagrin drop from Ekphoras, héraut du chagrin.

With 3 other players, stand on the corners of the platform and cast Convocation de la souffrance to summon these rares.

Exos, héraut de la domination is located on Altar of Domination (upper level) - The Maw.
To enter on Altar of Domination you need open a Gate in Perdition Hold.
To open the Gate you need the achievement Bonjour les ennuis ! (click on this achieve for more info).

After open the gate, run to the rare location on Altar of Domination and use Appel de la domination on the center of the room.

/way 46,0, 47,7 center of the room

After use Appel de la domination will appear a portal on the center of the room.
Enter on this portal to reach Altar of Domination (upper level) and kill the rare.

After completing the Necrolord Assault, used the portal in the chamber to the Apex, Exos was just sitting there waiting for me to give him a spanking

With Drakas horn, got some Prime Gatecrasher help but Solo'd as Warrior Tank, got the drop
