After turning this in, a city wide speech in Shattrath begins, just like Onyxia's head. It is as follows:

<Silence descends upon Shattrath>
<A'dal's thoughts invade your mind>
<Kael'thas Sunstrider has been defeated by (player) and his allies>
<The time to strike at the remaining blood elves in Tempest Keep is now. Take heart and let A'dal's song of battle empower you!>

Everyone in TK gains a buff called A'dal's Song of Battle which gives 10% more rep and 5% extra damage and healing.

Kael then appears in front of A'dal and shouts out a speech:

Kael: Your monkeys failed to finish the job naaru. Beaten, but alive...The same mistake was not made when we took command of your vessel.

Kael: All for what? Trinkets? You are too late. The preparations have already begun. Soon the master will make his return.

Kael: And there is nothing you or that fool, Illidan, can do to stop me! You have both served me in your own right - unwittingly. Lay down your arms and succumb to the might of Kil'jaeden!

This item explains why in the pictures of him for the Magister's Terrace, he only has 2 orbs floating around him instead of 3

When you kill Nef, Ony, or Mag, you take their heads as a trophy. When you kill Zul'jin, you take his blood.

Am I the only one disturbed by the fact that when you kill Kael'thas, you take his balls?

Don't care if this gets downrated, just like the time somebody opened a portal to theramore after a naxx run, and somebodys addon just spammed in raid chat <Busted> Portal to Theramore has been opened. Was epic win, like this.

If you plan on turning this in and wish to see the event that unfolds, wait for any other NPC RP event to finish first. I turned it in when the Blood Knights were pledging loyalty to A'dal and when the event was over, the quest one did not trigger.

i was doing the chain to get to get good with the oracles and this appeared in my inventory i kid you not and i was thinking wtf is this item, i mean i turned in the quest cause it was mine but still it was weird i mean i was doing it on an 86 priest and i don't even need the item i mean what would you do something like this had never happened to me before so i shrugged and just turned in the quest.

So, on a nice day I decided to start farming Kael.

First kill, no mount. I chopped him up with the Gin-Ji Knife Set, as I always did with any humanoid/beast that didn't drop what I wanted.

Then I returned to Shattrath to turn in the quest, and then Kael showed up and was all 'Your monkeys failed to finish the job naaru. Beaten, but alive...'

Apparently, you can come back even as a pile of dead meat if someone took one of your balls.
