Reaching Rank 5 with a bodyguard gives a quick quest that leads to a chest. One of the rewards in the chest are the Fourreau de cérémonie ankoïen/Fleur de mer éclatante, which grants 400 Reputation with the Waveblade Ankoan/Unshackled.

Reaching Rank 20 with a bodyguard has them give you a token of their appreciation. This reward differs from bodyguard to bodyguard:
Chasseur Akana/Néri Tranchécaille grant a snapdragon mount, Mordragon des fonds coralliens/Mordragon de varech;

Maître-lame Inowari/Poën Seldemer grant a Hermit Crab pet, Carapace-luisante brillant/Carapace-luisante lustré;

Long-voyant Ori/Vim Saumecœur grant a toy which transforms you into an Ankoan or a Kelfin, Charme ankoïen sculpté/Livre des Affranchis.

Each bodyguard has a series of achievements for ranks:
One for Rank 10 (example: Chasseur aguerri Akana);

One for Rank 20 (example: Vétéran Néri Tranchécaille);

One for Rank 30 (example: Maître-lame chevronné Inowari);

This achievement is part of the Nazjatar meta achievement, Usurpateur des mers, which awards the title and the Carapateur claquedos mount.

Rank 1: Perle de rêves lucides - Obtained by completing the quest Un répit de courte durée/Retour à Nazjatar (get a Bodyguard to Rank 3).
Major Effect: Souvenir des rêves lucides - Clear your mind and attune yourself with the Heart of Azeroth, increasing your Spec Resource generation rate by 100% for 12 seconds. (2 minute cooldown)

Minor Effect: Your spells and abilities have a chance to refund 50% of the Resource spent on them and heal you.
