This item will have a chance to drop from a Stèle du vice saccageuse (can drop from other mobs, also received one from a Nervi infâme or Massacreur de l'excavation) after you complete the small quest line that starts from Cloche du souvenir, which drops from the same monster.

Using it in front of the Seal of the Forgotten at the Crypt of the Forgotten summons a Forgotten Soul that you will have the opportunity to judge.

Sending it back to the crypt does just that, sends it back into the crypt. However, sending the soul to the Accuser awards you a Sacoche des héritages oubliés. Mine contained a random buff item and some grays.

My covenant is Venthyr, I am unsure if that matters.

first of all, you don't seem to get anything SUPER exciting for this. no court of harvesters or avowed rep, and nothing super thrilling from the satchel so far. the bells drop very quickly for me each day, in under 10 kills, and usually more like 3 or 4, so if you've been farming the rampagers for a while and gotten nothing, you may not be eligible for the quest for some reason. (because the mobs for the quest drop sinstone fragments, and the satchel can award them, this MAY be tied to the avowed faction. if anybody gets this without unlocking the avowed, and also if anybody does this questline as a non-venthyr character, please comment!)

loot Cloche du souvenir from Stèle du vice saccageuses. i have also had one drop from a Collectionneur infâme, so it may come from mobs west or east of the crypt (the mobs by the excavation are much more plentiful, so that may be a better spot for farming). i'm unsure if there are further requirements for this. the droprate for all the bells (not just this one-time quest item) currently seems VERY low, so it may be tied to the venthyr covenant and/or completing revendreth or avowed questlines.
the first bell you loot starts La cloche du souvenir. take it to the Seal of the Forgotten (/way 73.2, 33.8 near the venthyr-only greedstone in northeast revendreth) and meet Garde-vice Mourana.
mourana will give you Garde-vice and you will get a new, different version of the bell, this time Cloche du souvenir. use the bell in front of the crypt and a soul will come out for you to judge.
now you can go back and loot a THIRD version of the bell from Stèle du vice saccageuses, this time Cloche du souvenir. the steps are the same, although this version of the bell doesn't start a quest like the first one you looted. just take it back to the crypt (again, /way 73.2, 33.8), use it, and judge the soul that comes out.

you can either judge the soul as too arrogant to proceed or decide that they have improved greatly and can move on to the next stage. ~~if mourana agrees with you, she'll give you a Sacoche des héritages oubliés. if she disagrees, she'll tell you it was an "unorthodox decision," but she'll trust you (but you'll get no satchel).~~ i just got a satchel upon making the wrong choice, so maybe it's random? it's definitely not linked to whether you send the soul back in or let it move on, either (i've gotten it from doing both). i *think* this is daily (although you can do everything on this list the first day, i haven't been able to loot more than one bell per day since then). a tracking quest goes off when you loot the bell (#61194), which is what makes me think it's daily.

i'll come back and add correct judgments here whenever i can:
ARROGANT

Got a Sacoche des héritages oubliés that contained:
2x Grey items, worth about 15.75g
1x Médaille de vaillance abandonnée which is applied immediately and replaced with a Décoration inerte
1x Protège-bras d’humanité évanescente
